Pay toilets were relatively common in the U.S. until the mid 1970s, when a variety of factors (including a campaign by the Committee to End Pay Toilets in America) made them disappear. They can still be found in Europe.
Pay toilets had a door which could only be unlocked by inserting the appropriate coin. But to prevent people from being completely hidden within the toilet there was usually a gap under door. With the right amount of braveness or desperation the minimal payment could be avoided by simply crawling on one's belly along the bathroom floor.
