Fill baby bottle with formula and attach nipple. Stand it in a small saucepan, fill pan with enough water to cover the lower half of the bottle. Heat SLOWLY (water should only simmer) and test temperature of milk periodically by squeezing a few drops onto your wrist. The formula is ready when it feels just barely warm - 95-100 degrees Fahrenheit. (This is important - if the formula feels hot, it's too hot for the baby and will need to cool down a bit.)
When formula is ready, remove bottle from saucepan, pick up baby, and feed.
